Community Grants

Proposal Criteria

Our funding decisions are strategic and contribute to solving or effectively addressing the following health care issues in a substantial and sustainable manner.

Organizations should show how the project/program leads to a meaningful impact, is evidence-based or evidence-informed, and can be sustained beyond the funding cycle.

  

Pilot and more comprehensive projects are eligible for consideration.

Proposals require the following elements:

​

  • Project name;

  • Executive summary about the organization with emphasis on previous successes and current capability;

  • Population to be addressed;

  • Specific change(s) or outcomes to be achieved;

  • Background information about the population to include significant and relevant statistics and disparity data;

  • Clear statement of the strategy or implementation process with evidence-based or evidence-informed support;

  • Clearly defined and attainable goals and milestones;

  • Evaluation and performance improvement process that takes into account any potential unintended circumstances;

  • Realistic budget for the full project; and

  • Listing of staff, volunteers and/or community members who will be engaged in the project with relevant qualifications.

 

Preference is given to projects that also positively impact Prisma Health Richland's performance (e.g., decreased admissions, readmissions, preventable emergency room visits, and length of hospital stay).

Metabolic Syndrome

with a focus on pre-diabetes, diabetes, hypertension and/or obesity.

Immunizations

for priority and underserved populations like older adults and rural communities. 

Telehealth/Telemedicine

to create greater access for screenings, diagnosis, treatment and health literacy.

NOTE: Telehealth refers to both clinical and remote non-clinical services, including providing training and health education. Telemedicine solely refers to remote clinical services.

Social Determinants of Health

focused on addressing the foundational social, economic, and environmental factors that impact health outcomes, aiming to create equitable opportunities for well-being and reduce health disparities within the community.

Healthy Children

to foster holistic health and development and ensure access to resources that promote their physical, mental, and emotional well-being from an early age.

Healthy Adults

to promote the comprehensive health and well-being of the adult population, empowering them to manage chronic conditions, access preventive care, and lead productive lives.

Community Impact Partnerships

to support care beyond the hospital that focuses on access, chronic disease prevention, dental and vision care, emergency room avoidance, infectious diseases, mental/behavioral health, trauma and injury prevention, and pharmacy support.

Funding
Guidelines &
Restrictions

Selected proposals are funded based on the availability of funds as determined annually by the Richland Memorial Hospital Board of Trustees budget and may be approved for multi-year projects.


These grant funds may not be used for:

  • Physician, dentist, pharmacist or other healthcare worker salaries;

  • Salaries other than those approved to carry out the project;

  • Capital campaigns;  

  • One time or annual events, including sports tournaments, camps, or event incentives; 

  • The purchase of tickets or tables, membership drives, raffles or auctions;

  • Political campaigns; 

  • Activities designed to influence legislation; 

  • Advertising separate from project-related advertising activity included in the proposal and explicitly approved for funding;

  • Religious activities other than those included in the proposal and explicitly approved for funding; 

  • Indirect costs associated with the project other than those that are included in the proposal and explicitly approved for funding; and

  • Other grant-making foundations.

Though invited to apply, the Board’s Grant Awards Committee reviews all proposals received. To promote future success, constructive feedback will be provided to unsuccessful applicants.
